Со почетокот на КОВИД пандемијата, исклучително се форсираше кон вакцината како единствен спас, единствена превенција за оваа болест.
Како што времето одминува, сѐ појасно е дека вакцината не штити како што беше ветувано, а вакцината нема никаква улога во спречување на трансмисијата на САРС-КоВ-2, што впрочем и не беше дел од клиничките истражувања на производителите на КОВИД вакцините. Сега рекламата, т.е. вакциналната кампања се сведе на тоа дека вакцината штити од тешка форма на болеста, односно ја редуцира стапката на хоспитализација.
Протоколите за лечење, но и за профилакса беа и сѐ уште се во сенка. Известувавме уште од почетокот на пандемијата за лекови и суплементи кои би помогнале како во профилакса, така и за третман.
Уште на почетокот од пандемијата се говореше за улогата на витаминот Ц. Д-р Ченг, говореше за успешен третман со интравенозен витамин Ц и хепарин (кај одредени пациенти).
Известувавме за улогата на хидроксихлорокинот и цинкот како дел од успешен тратман и профилакса.
Ивермекинот се покажа доста ефикасен (1,2,3,4)
По две години се востановија доста успешни комбинации од лекови и суплементи за поддршка на заболените, но и за профилакса. Секако доказите само се трупаат (студии подолу).
За разлика од другите држави, во Македонија, сепак не постои ниту официјален протокол, ниту официјални насоки за профилакса. Здравата исхрана, редовната физичка активност „немаат влијание“ на добриот имунитет. Остана вакцината како единствен спас, но како што гледаме вакцината ниту спречи инфицирање, ниту помогна за враќање во „нормала“, а несаканите реакции кај вакцинираните се зголемуваат, иако Министерството за здравство не објавува конкретни податоци.
Многу доктори, како и практичари, оние кои имаат искуство со самата болест, врз основа на тоа искуство составија протоколи за лечење, како во домашни, така и во болнички услови.
Зошто не се се разговара за овие протоколи кај нас, зошто воопшто не се разговара за нив, зошто сите студии кои не одговараат на наративот се цензурираат – останува да видиме. Но, овие прашања се доста важни, бидејќи можноста за спасен живот плус останува на плеќите на одговорните.
Во продолжение може да видите дел од тие протоколи, како и научните докази позади нив. Заедничко за сите, но и есенцијален дел е раниот третман на болеста.
Доказите се акумулираа многу рано во пандемијата дека употребата на секвенционирани терапевтски средства со повеќе лекови (sequenced multi-drug therapeutics -SMDT) под водство на лекарот е корисна и дека некои лекови се безбедни и ефективни. Се однесуваат на пренаменети терапевтици кои се одобрени и се користат во некои случаи со децении за други болести.
Постојат опширно напишани и објавени алгоритми и протоколи за третман, како и докази за користа од раниот амбулантски третман на вирусот САРС-КоВ-2 и последователната болест КОВИД-19 (1, 2, 3, 4, 5, 6 ). Со високо насочени и SMDT режими кои вклучуваат рана примена на антивирусни лекови, во комбинација со кортикостероиди и антитромбоцитни/антитромботични терапевтици, ризикот од хоспитализација е значително намален за дури 85 до 90%, а ризикот од смртноста е елиминирана кај пациенти со висок ризик и кај помлади поединци кои се манифестираат со тешки симптоми.
КОВИД-19 се манифестира или како блага состојба слична на грип или како посериозна болест кај оние со висок ризик. Кај доста мал дел од лицата инфицирани со вирусот напредува кон посериозно заболување (обично постари лица со основни медицински состојби, гојазни или помлади со основни медицински состојби/фактори на ризик).
Комплексната и повеќедимензионална патофизиологија на болеста КОВИД-19, вклучително и оштетување на органите со посредство на вируси, цитокинска бура и тромбоза, налагаат рани интервенции за да се решат сите компоненти на болеста.
На кратко, болеста вклучува три фази 1) почетна фаза на вирусна репликација при што вирусот ја „киднапира“ метаболичката машинерија на клетките кои потоа почнуваат да синтетизираат нови вирусни честички 2) понапредна инфламаторна хипер-нерегулирана имуно-модулаторна бурна пневмонија, фаза при што постои цитокинска бура и проблематична размена на гасови позната како синдром на акутен респираторен дистрес – ARDS. ARDS генерално е причина за повеќето смртни случаи кои се припишуваат на КОВИД-19; и 3) тромботична фаза на згрутчување на крвта при што микротромбите се развиваат во белите дробови и во васкулатурата, што доведува до катастрофални компликации вклучувајќи длабока хипоксемија, мозочен удар и срцев удар.
Идеалната ситуација е да се запре вирусот во почетната фаза кога симптомите штотуку се појавиле, додека пациентот е сè уште во домот. Целта е да се спречи хоспитализација и смрт.
Во земјите каде што има и имаше неподготвеност за рано лекување на инфицирани и симптоматски високоризични лица, како што очигледно е случај кај нас, овој терапевтски нихилизам резултираше со ескалација на симптомите, одложена нега во болница и смрт.
Вирусни болести како што е КОВИД-19, со сложена патофизиологија, не реагираат на еден третман со лекови, туку бараат пристап со повеќе лекови. Вирусот е потребно да се третира со повеќекратни терапии. Овој повеќестран терапевтски пристап вклучува 1) адјувантни додатоци во исхраната; 2) комбинирана интрацелуларна антиинфективна терапија (антивирусни лекови и антибиотици); 3) инхалирани/орални кортикостероиди и колхицин; 4) антитромбоцитни агенси/антикоагуланси; 5) супортивна нега вклучувајќи дополнителен кислород, мониторинг и телемедицина. Рандомизираните испитувања на индивидуални, нови орални терапии не дадоа ефективни алатки.
Ниту една единствена терапевтска опција досега не била соодветна, но комбинациите се многу успешно применети во клиничката пракса. Лекарите кои беа храбри и сметаа дека е итно да се примени пристапот SMDT универзално, за да имаат корист голем број на акутни пациенти со КОВИД-19, го намалија интензитет и времетраењето на симптомите и спасија многу од хоспитализација и смрт.
Бидејќи во почетниот стадиум, а и со појавата на новите варијанти, е многу тешко да се разликува дали се работи за САРС-КоВ-2 или друг респираторен вирус (секако и тестовите не играат улога) – клучот е употребата на ран третман веднаш штом се појават симптомите кога вирусот е рана во фазата на репликација.
Д-р Џозеф Меркола вели:
„Со оглед на несигурноста околу дијагнозата, најдобро е да се третира секоја настинка или грип со појавување на раните симптоми. При првите знаци на симптоми, започнете со третман. Можеби се работи за обична настинка или обичен грип, можеби е многу поблагиот Омикрон, но бидејќи е тешко да се каже, вашата најдобра опција е да ги третирате симптомите како што би ги третирале претходните форми на КОВИД. Со оглед на тоа колку е заразен Омикрон, голема е веројатноста дека ќе го добиете, затоа купете го она што ќе ви треба сега, за да го имате при рака доколку/кога се појават симптоми. И, запомнете, ова важи и за оние кои се вакцинирани исто така, бидејќи имате исто толку шанси да се заразите – а можеби и уште повеќе“.

The cohort consisted of patients aged ≥65 years presenting with symptoms consistent with COVID-19 (n=105). COVID-19-positive arm demonstrated lower median serum 25(OH)D level of 27 nmol/L (IQR=20-47 nmol/L) compared with COVID-19-negative arm, with median level of 52 nmol/L (IQR=31.5-71.5 nmol/L) (p value=0.0008). Among patients with vitamin D deficiency, there was higher peak D-dimer level (1914.00 μgFEU/L vs 1268.00 μgFEU/L) (p=0.034) and higher incidence of NIV support and high dependency unit admission (30.77% vs 9.68%) (p=0.042). |
